The dark-brown dorsum of the adult Peaks of Otter salamander is distinctive, with extensive brassy flecking running the length of it. The centre of P. Haubrich is not marked and ranges in hue from light to charcoal grey. The dorsum and sides of the species may also have a few faint white spots. Small mental glands are located beneath the chin in males.

The Cheat Mountain salamander, to which the Peaks of Otter salamander is superficially similar, has a higher concentration of metallic flecking on its dorsum, and it typically has 19 costal grooves as opposed to P. nettingi's variable 17–19 costal grooves. Petranka claimed that hatchlings have a characteristic dorsal stripe made up of reddish dots, but more than 100 neonates that were inspected showed a homogenous dark-grey dorsum and no evidence of this.

About 25 mm from the snout to the vent, salamanders start to develop brassy flecks.
