Well in order to find the equation you are looking for you first must find the slope of the line. Since it gives you two points at (-4, 1) and at (0, 4), use these points to find the slope.
Slope = (change in y)/(change in x) = (4 - 1)/ (0 - -4) = 3/4
Since you already know the y intercept at (0, 4) you can now put these values in to get your equation.
y = mx + b where m is slope and b is y-intercept.
y = (3/4)x + 4
